LINK Healthcare is a growing and innovative healthcare solution located in Schofield, Wisconsin, providing direct primary care. Our clinic offers a variety of services, including primary care, occupational health, chronic disease management, and wellness programs. We're looking for a compassionate and forward-thinking individual to join our team as a full-time Health Coach. Health Coaches play a vital role in empowering patients to achieve their wellness goals through education, support, and personalized care. Requirements:

What you would do:

  • Conducts initial and ongoing consultations, performing comprehensive lifestyle assessments for each patient.
  • Offers verbal and written guidance aimed at mitigating unhealthy decisions and behaviors.
  • Utilizes assessment data to conduct analysis and devise individualized care plans tailored to each patient's specific needs.
  • Designs and delivers seminars for clients and participants.
  • Utilizes motivational interviewing techniques and effective communication skills to foster maximum participation and engagement.
  • Provides personalized coaching to empower patients in making independent decisions regarding lifestyle changes to enhance overall health.
  • Demonstrates the capacity to recognize patient limitations and discern when external referrals for ongoing care or counseling are warranted, drawing upon knowledge, education, and experience.
  • Adheres to established program workflows and proficiently documents client data in electronic charts in a timely manner.
  • Engages in both face-to-face and virtual telephone consultations and treatment sessions.
  • Demonstrates strong verbal and written communication abilities.
  • Ensures client confidentiality and safety while exhibiting compassion in assisting others.
  • Undertakes additional duties as necessary or specified.
  • Willingly conducts on-site visits with employers to facilitate health and wellness promotion among employees.
  • Desired experience in project planning and wellness challenges, though not mandatory

  • Demonstrates the ability to maintain accurate charting and record-keeping in a timely manner.

  • Maintains a professional appearance and demeanor, along with a positive attitude and confident demeanor.
  • Willingness to conduct on-site visits with employers and engage with employees to promote health and wellness.
  • Demonstrates excellent verbal and written communication skills.
  • Exhibits outstanding interpersonal abilities.
  • Displays exceptional organizational skills.
  • Previous experience in health coaching is highly preferred.
  • Possesses the capability to uphold HIPAA compliance and safety regulations.
  • Bachelor’s Degree or equivalent professional experience in a health-related field, such as nutrition, counseling, fitness, health education, psychology, wellness, nursing, or a related area of healthcare.
